Kentucky Homeplace, WellCare partnership shows significant growth in health benefits

Eastern Kentucky residents were participants in a yearlong program that highlighted benefits of increased access to healthcare. Workshops promoted preventive care and chronic disease self-management.

The workshops were run by specially trained community health workers who advocated for member access to medical, social, and environmental services.

Data showed a 10% drop in emergency room visits and a 23% decline in hospital admissions. Participants were also offered services that included health literacy, food and utility assistance and gas cards.

Direct education and support from healthcare providers, as well as removing barriers to transportation, allows for quality healthcare and better condition management.
