Anthem Blue Cross and Blue Shield in Kentucky has named Family Practice Associates of Lexington one of 44 primary care practices nationwide—and one of three in Kentucky—recognized through its second annual Care Provider Recognition Program.
Anthem selected the honorees from thousands of primary care practices in its national network based
on measures including preventive screenings, immunization rates, chronic disease management,
medication adherence and other patient health outcomes.
The program recognizes primary care practices that work with Anthem to deliver coordinated,
personalized care designed to improve health outcomes and the patient experience while helping lower
health care costs.

Applegate, president and managing partner of Family Practice Associates of Lexington. “FPA has a long
history of providing coordinated care focused on wellness and effective management of chronic
conditions. Our teams work to ensure patients receive recommended screenings and can overcome
barriers to managing chronic disease.”
